Services After Discharge

Your doctor will determine the day of your discharge and issue any necessary orders.

Even after you leave Peconic Bay Medical Center, our commitment to your health care needs continue.

Through the Discharge Planning Program, all our patients' needs - medical, emotional, and social are assessed.

Should this evaluation indicate the need for special services after you are discharged, such as home health care or nursing home care, these issues will be discussed with you and your family and arranged by our discharge planning and social services staff.
